How to set up outputs as boundary condition

I am simulating a coupled free fluid-porous medium flow. I did a simulation with CFD module gaining the values of the pressure along the bed of the free fluid domain. I need to set the values of pressure as boundary conditions for the porous medium. How I can get this ?
